PRCUA Education Fund Luncheon Registration Form

Dear PRCUA Society Officers and Members:

You are cordially invited to “Hats Off to Our Scholars” fundraiser held on Thursday, August 7, 2008 in the PRCUA Social Hall located at 984 N. Milwaukee, Chicago Illinois. With the high cost of education more and more PRCUA young adults are looking for scholarships to help defray the cost of their higher education. All proceeds from this luncheon will go towards the PRCUA Education Fund, Inc. Information on the PRCUA’S Education Fund is enclosed.

Please join us (ladies and gentlemen) for an afternoon of good food, fun, door prizes, entertainment and a raffle. Dress up with your favorite hat! (The men also have some interesting hats.) Prizes for the most unique and most elaborate hat will be given. Bring your friends (with or without hats)! If you wish to sit with a particular group, please list their names on the registration form.

Doors open at 11:30 a.m. with lunch being served at 12:00 p.m. Cost is $25.00 per person. Deadline for reservations is August 1. For further information, call Alice Robaczewski at 773-782-2610.
